Citation formatting is based on the citation style that you are using. Use the Document Preferences button in the Zotero toolbar and check to make sure you are using the correct citation style.

Do you have any other plugins besides Zotero installed in Word? If so, could you see if it works with them disabled? Also, after inserting a citation, are you not getting a popup that says an error occurred in the Zotero word processor integration?
