Do you get errors for bibliography inserts in a fresh document? Can you find your newly added items in a fresh document? If not, can you find them if you click on the Z in the dialog and switch to the classic citation dialog?

Try updating translators in Zotero Preferences -> General and after that resetting translators in the Chrome extension Preferences -> Advanced.

The errors you posted above are not related to Word integration. Did the bibliography get inserted successfully? Did you get an error prompt? What did it say? What do you mean by the citation window "not seeing" your items?

If you double click on Zotero.dotm (described how to find it in the manual installation instructions), do you get the toolbar? As Gracile pointed out, you should look to update Word 2016 to the latest version available.

In the Developer Tab when you click Macros do you see Zotero related ones and if you click on them do they execute properly? Did the add-on in word worked properly before?

This Debug ID however shows a successful update of the document. Does it only crash sometimes? Do you get any other error messages when it does?

An update to one of them could have caused this. Try disabling those to make sure they're not the culprit first.

Oh, right. I missed the part about the text box. Yes, that's never worked and won't work for the foreseeable future as it's a limitation within LibreOffice that's stopping us (although we could perhaps have a better error message).

It's been moved to the Zotero Connector extension for the browser. Found under Preferences -> General and is enabled by default.
