The zotero:// links I mention can already link to a PDF file in Zotero or even specific annotations that you have made there. Can you clarify which feature you are missing?

As I tried to reproduced the steps I describe above, I realize that it does not always work. To make it work, it has to load the PDF preview of Item B before creating the new Item A. I have modified the post above with the additional steps.

Can you reproduce this in Troubleshooting Mode (Help → "Restart in Troubleshooting Mode…”), which temporarily disables all plugins?
