I have used this ADD-on and set a pdf as default but it didn't work.The default one is still unchanged.

I wanna know about the rules for definding a default pdf.Is it defined by the time sequence of pdfs putting in an item? which means the first one may be the default pdf.

Yes, currently the first-added PDF is used. We plan to make it possible to choose another attachment in a future version.

Is the manual method simply to create the record again and then add the desired pdf in first?

While we wait for the final fix, there's a possible work-around using the Actions & Tags plugin (more actively maintained than zotero-pdfkit), as discussed here https://github.com/sharpevo/zotero-pdfkit/issues/3
