New PDF reader - Add Item Note from Annotations produces an empty Untitled Note

It's all in the title:

"Add Item Note from Annotations" produces an empty "Untitled Note" instead of collating my annotations. Was working OK 30 min ago. Any ideas about why this is happening and how to fix it?

The new PDF viewer and annotation features are are superb development!
