zotero tool bar does not appear in word for mac 15.13.3

I have followed all the instructions in zotero 5.0 page, i can see zotero.dotm is loaded in word, Zotero Word for Mac Integration is fine but the add in table does not appear in Word. Any thoughts?

If you open the Zotero.dotm file directly, does the tab appear? What macro security settings do you have set?
