errors with vancouver style

There are numerous errors in Vancouver style: spacing between text and punctuation, failure to include location in reports, etc.

    I don't think anyone ever bothered with reports for Vancouver - you can try the NLM style from the repository, which might be better, but not sure about reports there, either. I thought the spacing was mostly correct, though.
    Otherwise, how about providing some more detailed feedback rather than "numerous errors" which is not going to help anyone.
