I just realized what the problem is. I can't tell what state I'm in: is the ToC focused or the document? If I press the down arrow am I going to lose my place? Proposal: when the ToC is focused, highlight it in a different color.

Sometimes there is no "Save to Zotero" option in the menu that appears from right-clicking on the extension. Only the generic extension options.
