I did not have tracking enabled as this was happening. You see tracking in the image above because this is how the SharePoint displays version history. No, I cannot reproduce this.

Well, the reference was of a book with two editors, one incorrectly entered as an organisation by the connector. I added to Word and noticed then. Deleted the editor and added a new one. Got the error. Tried several times. Nothing was visible in …

OK, I fixed it by deleting temp files, restarting the computer. After that, I could see multiple entries of the offending citation in Word. Deleted them, restarted Word, and could add again as expected. Something weird in Word.

Hm... By this logic even if I submit a PR the bottleneck will remain because testing if the PR breaks something will also be laborious. How can we have it fixed given this is one of the most common styles and known bugs in it are a problem?
