I actually prefer that the outline be collapsed by default. For me, it's quicker to find a specific entry by first selecting the chapter, then section, etc. instead of scrolling around.

I've had luck shrinking large margins by modifying the CropBox for each page in the PDF. The existing annotations still rendered in the expected locations.

Ah, that makes sense. My window manager has Alt-click-drag mapped to move windows, which could be interfering.

Sent. That PDF also has some citations that don't trigger a popup (Evince does show the popup). See the citations for Donoho 2006 on page 3.
