I'm not sure but I'd say the date added to zotero. I don't think Zotero saves the date the file was attached to the entry (but the "last modified" date of the file is updated when you move it to another parent item).

Not sure what you mean. It works fine for me. I was expecting this button would remove any formatting. But it does not remove links, quotes and styles (i.e. heading 1, 2, 3…). [Edit: "Clear formatting" had the same behaviour in 4.0]

Yes it doesn't work. It's a known issue. Save your report, open the saved file (it will be saved as a webpage which you could open in your browser) and you'll be able to search inside it.

Uhm you've edited your original post. In reports, it has never worked. You need to save it (File > Save, or Ctrl-F) and then search in the HTML file.

Click on the binoculars icon, it didn't exist in 4.0 (and searching was possible because of Firefox built-in function for Zotero reports only).
