The image is probably too big. Generally speaking, it's not a good idea, at the moment, to embed an image into a note (e.g.) You can attach it to the item, though.

Update to Firefox 20. What's you version of zotero ? http://www.zotero.org/support/kb/zotero_version You're using both Standalone and Zotero for Firefox on your windows machine, right?

It's probably an encoding issue. Instead of these characters, the name of the collection/saved search/library to which the selected items belong should be written.
