Zotero syncs the correct files. If they're not appearing, or they're showing as 404s, that's some problem with your WebDAV server.

@smteinturs: And you're at your Zotero Storage quota, so additional files wouldn't be syncing. Zotero would be showing a warning about that in the toolbar on every sync.

Make sure the document preferences or citation dialog isn't just appearing behind your other windows. That's the most likely thing here. If you're sure it's not that, we would want a Debug ID for reproducing this after restarting Zotero.

Selecting the attachments is only necessary for dragging. You can just use File → Export PDFs with the parent items selected. Export Items is for exporting metadata.
