I had already tried that (and just tried it again now). No luck. Firefox doesn't seem to "see" the .xpi file in that directory.

I re-installed *everything* from scratch and rebooted. Shouldn't have to do that, but it did eventually work.

The toolbar *does* appear - I've got the "Add-In" menu in Word and I can see all of the usual options (like to add a reference). But when I click on any of the options in this menu, nothing happens.

@Rintze: The missing ">" at the end was a copy/paste error from my .csl file into the git webform.

If remove the 4 lines inserted for sorting, then everything works perfectly and I get (un-sorted) entries with the appropriate italics. I can repeat this experiment.
