I have had such issue before (reported here), so I made sure to restart a few times to check that it did not actually update. However, I just got the beta.26 update after manually checking, so this may not be an issue anymore.

Attachments aren't cited. Items are. Deleting an attachment under an item should not affect any of your citations in your word processing documents. You can place many attachments under an item in Zotero, delete them, move them and remove them.
