Were you importing a lot of articles in a short period of time? arXiv implements some rate limiting that we may be ignoring, which could have caused this error (though I think it would have looked a bit different).

Yeah, we would need a debug log for a failed attempt. I would guess that it was some intermittent error/outage with their OAI server (though I don't think we've had reports of this before)

The only way that is possible is if your library is not syncing at all and you added a PDF to an item on a different computer that is syncing. See last three paragraphs here https://www.zotero.org/support/kb/changes_not_syncing
