This needs to be cross-checked across versions of Word. Those guidelines do apply to LibreOffice and Google Docs too though. Google Docs doesn't really have captions, but it definitely causes issues in LibreOffice.

It's most likely the fault of LibreOffice 7.4. You could probably revert LibreOffice versions 5 years back and wouldn't see this behavior with the Zotero plugin, so something in the new version of LO is causing it. Perhaps it's new intended behavior…

Your shortcut should call ZoteroAddEditCitation. Your shortcut is now the same as clicking the Add Note button in the extension, which is used to insert notes, not items. See https://www.zotero.org/blog/#adding_notes_to_word_processors

Hm. This is highly unusual. It's most likely something with Word config. The quickest way to test that is to create a new Windows user account and see whether it works there.

Could you produce a Debug ID from Zotero for inserting a bibliography into a new document with one cited book with APA? Do you get italics in other word processors: LibreOffice or Google Docs?

Not all citation styles use italics in bibliography, and not for every item type. Try citing a book with APA7. The book title should be italicized.
