Restart your browser. Zotero should prompt you for those permissions again.

You should edit the author field of the item that is causing you issues in Zotero and make sure there are no empty spaces or tabs in it. Then press Refresh in the Zotero tab in Word.

Can you explain in more detail what you're trying to do and what happens? Do you run any security software and did you try temporarily disabling it?

Did you make sure Zotero and Word run as the same user and neither run as Administrator? Are you using Windows? Did you try restarting your machine?
