Did you try restarting your computer? If you go into Tools -> Add-ons in Zotero do you see the Word addon? Is it enabled?

Are you citing with endnotes? Which citation style? How long is the document? Do you have tracked changes that are not accepted in the document? Can you reproduce this in a new document?

Have you tried restarting Zotero? If it doesn't work after a restart could you produce a startup Debug ID from Zotero?

Inserting in new documents is relatively fast. Does it happen after restarting Zotero and your browser?

What were you doing when it disappeared? Does it reappear if you restart Safari? Is is still present in Safari Preferences?

Did you try restarting Zotero and Word? When you click a button in Word did you try looking through Zotero windows to make sure the citation dialog isn't behind other windows? Which OS and version of Word is this?

Is this when pressing a button in the integration like Add/Edit Citation? Could you try reinstalling the document plugin from Zotero Preferences -> Cite -> Word Processors, then restart Word and try again?
