I just had a similar problem, but after going to Firefox>>Tools>>Add-ons>>Extensions>>Zotero LibreOffice Integration 3.5.2>>Preferences>>Cite then click on Reinstall. I can now see toolbar and icons.

All set. Thanks! I can't remember what I did in setting up this computer last month, but I know I had problems with Open Office connecting with Firefox/Zotero and found some work around.
