You mean affiliations? That's just not part of Zotero's data model, which focuses on citation metadata.

Add it after the first name preceded by a comma so: Smith, John, Jr. This does appear to put a comma before the Jr. in the bibliography (so Smith, John, Jr.) but the rest will come out correctly.

You can certainly have more than one attachment on an item and see that online (I can't speak for the web library). What do you mean by "the others have individual locations"?
