That's standard behavior for footnotes/endnotes (Word will only insert them with consecutive numbering). If you want repeated numbers for identical citations, you'll want a 'numeric' citation style like Nature or Vancouver among those preinstalled.

That's correct Chicago Manual since the 18th edition. Are you sure you need it (there are some style options that still include it, but for common usage you shouldn't need it)
