Do you have any plugins installed? I don't think Zotero has any functionality to add metadata once a parent item exists (various plugins, e.g. Zotero linter, do).

The APA Manual is explicit in discouraging the use of access dates as long as a date of publication is available. That's what the style does -- you can use a different citation style (easy) or edit the existing APA style (fairly involved).

That's just standard Word justified print: the long citation doesn't get split up because it doesn't have any spaces. You could likely add a space as a prefix/suffix to a citation somewhere in the middle there and get a line break that will look OK …
