The basic fix is now available and should auto-update for you within 24hs (doesn't require a new Zotero version). You will still get better metadata using the DOI, though.

If you are just seeing the name and not n.d., you are not using APA style. How exactly are you copying?; And are you sure you don't want to use the Word add on?

That's not a standard, it's a report. You you switch the item type and enter NCTQ as Institution (APA 7 generally doesn't cite the place.of publication)
