Any problem with an plugin should be reported to its developer. You probably would have more luck using the stable version rather than a pre-release.

SLEEP is a dependence of the American Medical Association 11th edition style, but it should now depend on the American Medical Association 11th edition (brackets) style. You can use the latter meanwhile.

Not sure how you installed the add-on manually, but you should have Zotero_LibreOffice_Integration.oxt, not Zotero_OpenOffice_Integration.oxt. Did you reinstalled it via Edit > Preferences > Word Processors > Reinstall LibreOffice Add-in?

I guess you already have tried it, but what happens when you disable all the add-ons and restart Zotero? Edit: Had the same problem just now. It is related to the "Zotero pdf preview" add-on.
