I am having the same problem using OS X 10.11.6 (15G1004). Deleting the zotero folder at ~/Library/Caches/Zotero/updates/Applications works temporarily but has to be re-done each time the problem occurs. It is a recent problem.4.0.29.14.

Just to add, even after saving, refreshing the citations has the same effect. Is it safe to continue working on this document?

Seem to find the http://sfx.coventry.ac.uk/coventry works for Coventry University. Took from the setting used by Google Scholar. I don't know if this is a good generic trick.
