I figured it out. I clicked on the 'hamburger' icon (far right), then Add-ons, then the gray circle gear icon (top middle right), and lastly "Check for updates". The Zotero Add-on updated t the most recent version and it started working again.

Thank you. The error has been resolved. The .Ink file did not appears as an attachment to the record. Deleting the record resolved the issue.

The blank output occurs when generating a report for a single reference. Would be nice to know if another Zotero user has similar problem, otherwise, the issue may not be the plugin itself.
