Multiple citation numerals lack commas or hyphens

I am editing a manuscript in Microsoft Word for Mac (16.114.3). My style is Journal of Occupational and Environmental Medicine. I inserted two citations after the following sentence: A substantial literature links PTSD to elevated risks of cardiovascular disease. Zotero inserted them correctly according to JOEM style as superscripts 1 and 2, but there is no comma between them, so it looks like the number 12. If I insert a third reference, the superscript then appears as 123. The 12 should really appear as 1,2 while 123 should appear as 1-3. What is wrong?

Are you entering these in th same cite bubble (red) or one by one? If the latter, they'll be just "123". Text after text after text.

If you've inserted them one-by-one, pressing the Refresh button should merge them and display them correctly.
