Notes and tags gone

My notes and tags have disappeared from the summary sidebar when clicking on a text. The notes and tags still exist in the document, but no longer are searchable in the main menu or show up in the side bar summary. There are also new, autogenerated tags for each document that I can't get to go away. 125412989

The notes and tags appear when I open the document, but not in the summary when I am on the home page viewing document titles.

Those are annotations on the attachment, not notes on the parent item. Click on the attachment item (or click the 11 there).
