Vancouver style with superscript bibliography numbers AND superscript page numbers
I'm preparing an article for submission to the journal BMJ Medical Humanities. It has recently gone over to Vancouver style, but to a peculiar variant that uses page numbers in the in-text citations. Citations appear in-text like this (all in superscript):

5(pp8-9)
and so on.
There is a ready made style NLM/Vancouver: Citing Medicine 2nd edition (citation-sequence, superscript). This gives the in-text bibliographic reference in superscript but doesn't give page numbers.
Does anyone know of an existing style that does what I need (hiding perhaps under an obscure name), or can anyone advise on how to modify an existing Vancouver style?
(Note: There is a Medical Humanities style, but this is out of date, and gives only the bibliographic reference number in square brackets, and no page numbers)
