Can you provide a Debug ID for Zotero startup through reproducing this, using the Help → Debug Output Logging → "Restart with Logging Enabled…" option?
Can you just post raw screenshots directly? Too hard to see that. Also, I'm not sure what's going on with your windowing — the citation dialog appears to have been made huge and is cut off at the bottom. Can you resize it to show the full window?
Ensure you are pressing on the "Add/Edit Citation" button in the toolbar. Based on what I can see on the blurry screenshot, it looks like you may be pressing "Add Annotation".
(But also resize down the citation dialog. The reason I was asking about that is because the dialog has a bottom toolbar with additional mode toggles, including one to switch between inserting citations/annotations/notes regardless of which toolbar button you press initially.)
