Attachments Added to Single Parent Item

I recently added a number of PDFs to my library. After syncing, all of the attachments -- and many existing attachments -- were grouped under a single parent item and were removed from the then-existing parent items. How can I solve this without changing the parent items?

    Can you provide a Report ID? That's likely something a plugin did — Zotero should never do anything like that.
