I notice the same: the Ctrl+Scroll breaks quite often in the latest beta. I haven't identified reliable reproducing steps yet.
When it is broken, it is not just a small time delay. I can continue scrolling and zooming in other tabs, then go back to the broken one and the Ctrl+Scroll is still broken. Making a simple Scroll without Ctrl makes it recover.

This problem is making the reader very painful to use. I need to make additional scrolls all the time to fix the broken zoom. Even the Ctrl++ and Ctrl+- are broken.
See a screen recording showing the problem. You can see at the beginning of the video that the Ctrl+Scroll shortcut is broken. Then it is fixed by making some additional scrolling. Then broken again, just making scrolling with the mouse (at around 19 seconds).

I have the same issue as described by OP - it has been present for weeks if not months in Zotero 7 Beta and still currently in Release v7.0.3.
I can reproduce the issue (in Windows 10) exactly as @pcinque describes. CTRL++ and CTRL+- always work as expected, but CTRL+MouseWheel frequently breaks.
@mjthoraval That's strange. I can reproduce various zoom-related issues in the original PDF.js. Zooming is intermittent when using either the mouse or trackpad, and sometimes it gets completely stuck. Occasionally, the zoom direction even reverses. I'm wondering if we are talking about the same issue.
Note: Cmd/Ctrl + Wheel/Trackpad zoom doesn't seem to be supported in Firefox's integrated PDF.js at all. It only works when using the link, at least on macOS.
It sounds like the problems you describe are different. The steps to reproduce are described above, and confirmed by other users. You can also see the screen recording.
When the zoom is not working, it will always stay broken until you release the Ctrl, scroll to move up or down, and hope that it will work when trying to zoom again with holding Ctrl. You can wait for a very long time, click in the PDF area, add annotations, resize the Zotero window, switch to another tab (where zooming with Ctrl works) and back: if you do not scroll without holding the Ctrl, the zoom will never work. It is not a temporary issue. It is always permanently stuck when the bug is triggered.
Yes, I see that on the original PDF.js link on both Firefox and Chrome browsers. It sometimes stuck completely no matter whether using mouse wheel or trackpad pinch zoom. So it seems to be the same problem.

@martynas_b If I use Firefox's integrated PDF.js, I see exactly the same problem as in Zotero. The Ctrl + Wheel zoom works sometimes, but can be broken in the same way as in Zotero. I never see the problem when using the link. The Ctrl + Wheel zoom is always working perfectly.
I have the same problem with the scroll wheel. Unabled to use the mouse to zoom in and out has confused me for a long time. Is there any solutions except for downgrading to Zotero6? Zotero6 works fine for me.
