Citations Inconsistent - APA 7th Edition - 1088521484

I'm having significant issues with my APA citations. Several different bugs seem to be occurring. One example is that at times the in-text citation will list two authors before "et al." : (Last, Last, et al., date).

I tried to re-enter the citation, just to see if I could fix it and it resulted in another error where the author's first initial was listed first in addition to the first error. This is also inconsistent with APA journal articles. For example: (R. Last, Last, et al., Date).

Can anyone help with this? It's happening for several citations, but not all of them. I don't know what the common denominator could be.

The first issue is disambiguating several citations with the same first author but different second authors (in line with the APA manual).

And just to be clear, both of these are correct according to the APA manual (be sure you have the names of the same author consistently entered across the items in your library).
