Some publisher feeds are incorrectly formatted and distributed in a way that causes this problem. I stopped using ScienceDirect feeds because of this. I notified Elsevier of this and received a reply from them. They said that repeated item distribution was intentional. I wrote back saying that defeated the purpose of how most people used feeds but the publisher continues that unfortunate policy.
