    I have followed the instructions, but unfortunately the problem persists.

    I am experiencing exactly the same problem. So far no remedy found despite deleting the 00000000 insert after the failure warning and inserting manually

    What exact version of Word are you using? Have you tried reinstalling the plugin from Zotero preferences?
