Citation not being added properly

I am using Mac OS 10.6.8, Firefox 3.6.22, with Zotero plug in 2.1.10, Word 2008 12.3.0 with Zotero plugin 3.1.16

When I try to place a citation, Zotero is placing 0/0/00 0:00 AM, which has the field code { PRINTDAET \* MERGEFORMAT }. The same action also places '7)' after other citations in my document. For example, (Adams et al., 2007) becomes (Adams et al., 2007)7). It's not generating an error, so I don't have a code to provide, but happy to provide log if needed.

In other instances, Zotero is placing {Citation }AM. When I toggle the field code, I see {ADDIN_ZOTERO_TEMP T}. It is also adding '7)' to the end of other citations, as in my original post. Both of these bugs have occurred on two different computers.

Do you have other plugins enabled in Word? (Check Tools -> Templates and Add-ins.)

Is there a solution to this problem? or has it been answered elsewhere?
I'm using Zotero in Word for Mac 2011. It has been working perfectly for the last year but is now inserting 00 before all my citations, any suggestions?

The problem has usually been a citation in a table or caption that messes this up. If you can't spot that easily, you'll have to run through
