It also seems that tags are duplicated: Some tags are listed as both automatic and manual.
This may be related: Quite often when I use Zotero Standalone for a while, and toggle the "Show automatic" option on and off for the tag selector, the checkmark fails to appear when automatic tags are shown. Toggling automatic tags will work, but it is just not updated in the UI.
I had debug level 3 enabled, but it does not show anything relevant. I am now running level 5 to see if that reveals something and will post here if I get something useful.
