I have same problem. Zotfile nor Zotero don't seem to delete the attachments in the cloud storage when I delete pdf item in my library and empty trash.
Zotero doesn't delete linked files if you delete the attachment items from Zotero, since the whole point of linked files is that you manage them yourself. If you want Zotero to manage the files, use stored files.
