multiple notes when it should be a single one

I wanted to add a footnote, but Zotero created three identical notes all numbered the same. I deleted the note and tried again, but to no avail. I get the same note...three times.

    1) Can you provide a Report ID?

    2) What word processor are you referring to?

    3) What citation style are you using?

    4) Can you take a screenshot of what you're describing, upload it somewhere (e.g., Dropbox or Google Drive), and provide a link here?
