references not syncing

I have added an additional reference into my word document but instead of adding to the bibliography and syncing inline with other references e.g. 41,41,43,44,45 It has not synchronised with the reference list and has started again e.g. 41,42,1,43,4445 using Vancouver referencing style.

Sounds like you flattened your citations. This can be due to several reasons, but most likely you saved it in a wrong format, or it got broken by one of your reviewers. See: https://www.zotero.org/support/kb/existing_citations_not_detected
