Zotero disappeared from Word

Hi,
i am on Mac Monterey 12.2, i have Zotero 5.0.96.4 and Word 16.58
Zotero is no more present in the menu bar.
i did what explained in the thread Zotero does not have the permission to control Word (zotero is enabled to control word), i tried to reinstall the plugin form inside Zotero, i restarted Word and also the Mac. nothing, Zotero not showing in Word.
any help?

i followed the instructions.
i have zotero.dotm in the word startup folder.
zotero appears only if manually add it with tools/Templates and Add-ins.
when i restart word, zotero isn't there

Are you saying that Zotero.dotm is unticked when you restart Word, or is it not even in the list of Templates and Add-ins?

In Word Preferences -> File Locations what is the location for the Start-up folder? It should be set to the default startup location/empty.
