Trouble inserting new numbered citations into Google Docs

Writing a paper using ACM style. When I insert new citations the numbering in the references doesn't update. Sometimes Google Docs inserts a new set of numbers at the top of the already numbered list. Any suggestions?

Upon further investigation, I believe this may be due to the fact that many of the references were unlinked at some point.

Yes, that would be the likely cause
