My document is not updating - have automatic updates disabled but when I push refresh it isn't working

That seems to show a refresh still in progress. You likely just have to wait longer.
Word for Mac is slower in general, and it looks like you're using both an older computer and a very old version of Word, so this could definitely be slow.
