Zotero missing from Word 16.53 for Mac

Hi, I was able to use Zotero with Word for Macs integration successfully in March 2021. However, I just noticed that the Zotero icon no longer appears anywhere in Word (even though Zotero is open), so I am unable to use Zotero for citations in Word. It's not clear to me when the Zotero icon vanished, as I hadn't tried to use it between March 2021 and today. Here are the currently installed versions on my old MacBook Air.

macOS Big Sur 11.5.2
Microsoft Word Office 365 version 16.53
Zotero 5.0.96.3 and 5.0.97-beta

I followed the following instructions without any success:

More specifically, I tried to troubleshoot as follows, yet each time I reopened Word, the Zotero icon is still missing:

1) Tools -> Add-ons -> Zotero Word for Mac Integration is enabled (there is clickable Disable tab)

2) Zotero -> Preferences -> Cite tab -> Word Processors -> Reinstall Microsoft Word Add-in (even though it already read "The Microsoft Word add-in is currently installed.")

3) Manually found the same Zotero.dotm in both Word startup folder and "install" folder. Also tried to copy and over-paste the existing Zotero.dotm file.

4) Word -> Tools -> Templates and Add-ins -> Zotero.dotm is present and checked

5) Restarted Microsoft Word, Zotero, computer.

6) Reinstalled Zotero. Tried beta version of Zotero. Restarted computer again.

I also generated a Report ID (434326831), but it's not clear if any of its contents are at all relevant for this missing Zotero icon phenomenon in Word.

I would hugely appreciate any insights to fix this issue. Many thanks for considering.

There's never a Zotero "icon" in Word. There's a Zotero tab in the ribbon. If Zotero.dotm is present and checked, you should see it.
