Zotero toolbar disappeared for Word 2004 on Mac

I had the Zotero (2.0 Beta) plugin for Word 2004 (on my Mac) installed and working perfectly. Recently I downloaded and installed the free trial of Design Sciences' MathType (6.0a). When I started Word, the Zotero menu bar was gone. I went to firefox and tried to reinstalled the plugin.. and other than spinning the color wheel for a few minutes nothing happens. When the color wheel stops spinning and I reload Word, the toolbar is still gone.

I went into Documents\Microsoft User Data and got rid of the Zotero folder. I know its suppose to be installed into Documents\Microsoft User Data\Word Script Menu Items but that folder did not exist when I initially installed the Zotero plugin.

Any ideas?

    In Word 2004, there should be a Zotero.dot file in /Applications/Microsoft Office 2004/Office/Startup/Word (the other location is for Word 2008). Have you checked View->Toolbars to see if the toolbar is available there?
