Upgrade StorageSouth African Medical Journal updated citation style

Would it be possible to update the citation style for the South African Medical Journal? (http://www.samj.org.za/index.php/samj/about/submissions#References):
1) First and last page, in full, should be given e.g.: 1215-1217 not 1215-17.
2) If there are more than six authors, the first three names should be given followed by et al.
3) DOI link should be included in all references, when available.
I tried to edit the style myself but beyond my skills I am afraid.
