    First, this could be due to security software on your computer preventing Zotero from updating the plugin. You can try adjusting any such security software so that installing the plugin from the Cite pane of the Zotero preferences works without an error, which will also allow future updates will work automatically. If you're not able to fix that, delete the above Zotero.dotm file manually, restart Word to make sure that the Zotero toolbar is completely gone, and then follow the manual installation instructions to reinstall the latest version of the plugin.
