Can't find "new item" button

Help! I can't find the green "new item" button for manually entering items anymore. The interface I see has a "+" sign for adding items. When I click it, it adds a blank entry to my library but does not give me options for entering any title, creator, etc. data into the entry. Seems like a very basic question, but I'm stuck. Any ideas? Thanks!

    Is this online or in the Desktop app? Online, you can just enter information on the right after clicking the + icon.
    In the app, it's generally the same idea, but the right-hand pane can be collapsed.
