"Dead item" on PC version

A new Zotero user has contacted me with a problem I have never seen. She has a PC and a Mac both syncing the same library. She created a book item with a PDF attachment on the PC. It synced to the Mac and worked fine there, but it went "dead" on the PC. She can't do anything to it, or delete it. She then deleted it from the Mac and synced. It did not remove it from the PC. What might have caused this and how can she remove the dead item?

    I'm not sure what that means, but we'd want a Debug ID for an operation that doesn't work and a clearer explanation of exactly what she's doing and what's happening.
